Opportunity for Flu Shots

Personnel from Marshall County Health Department will visit the Junior/Senior High and Marysville Elementary Schools on TUESDAY, Nov. 25, 2008 for the purpose of administering flu vaccine to staff and students who would like to receive them. If you are interested in this opportunity, please follow the instructions below:
-Please pick up (or have your child pick up) a CONSENT FORM, available in the offices of Marysville Junior and Senior High or Marysville Elementary.
-Complete and SIGN the consent form. Junior High and High School Students under age 18 will have to have a parental signature. Elementary student must have a parent present.
-Provide the completed and signed consent form at the time you come for the vaccine.
-Cost of the vaccine is $20 per person.
-If you wish for health dept. personnel to bill your insurance for the vaccine, PLEASE PROVIDE A COPY OF BOTH SIDES OF YOUR INSURANCE CARD AND BRING IT AT THE TIME YOU COME FOR THE VACCINE.
Students at MJHS/MHS can report to the NURSE’S OFFICE AT THE WEST END OF THE MAIN FLOOR OF THE JUNIOR HIGH SCHOOL BUILDING. Shots will be available that morning from 7:30 a.m. until 8:15 a.m. for students and until 8:45 a.m. for staff members.
Students and staff at Marysville Elementary school can begin receiving flu vaccine at the end of the school day @3:40 p.m. in the nurse’s office. Again, PLEASE have the consent form completed when you come for the injection.
